First Visit

We are excited to meet you! When you enter our practice, our priority is to help you feel comfortable and at ease. After a warm welcome by our staff, you will have your initial consultation appointment with your oral surgeon. This appointment will consist of a full examination, an explanation of your diagnosis, and personalized treatment plan options. Occasionally, a procedure can be performed the same day as the consultation. However, a more complex medical history or treatment plan may require further evaluation and a second appointment to provide treatment on another day.

You may also have diagnostic images taken during your first visit, such as 3D scanning or X-rays. If your general dentist has given you a written referral or X-rays, please remember to bring them with you to your appointment. Alternatively, you may request that your dentist forward the X-rays to our practice. All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

Please bring the following things with you to your consultation:

  • Surgical referral slip.
  • Any applicable X-rays.
  • A list of medications you are presently taking.
  • Medical and/or insurance information.
  • Driver’s license or other government-issued ID.
  • If you have medical or dental insurance, bring the necessary completed forms and cards.

At Dental Implant & Oral Surgery Specialists of Iowa, we are proud to provide the highest quality of care based on your individual needs. Please be sure to alert us to any medical conditions you have that may be of concern prior to surgery, including diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, and other significant medical conditions. If you are currently taking any medications (e.g., heart medications, aspirin, anticoagulant therapy, etc.), please let us know. If possible, we would like for you to arrive a few minutes early to allow time to complete the medical/dental health questionnaire, the patient information form, and the financial information form, if you have not previously done so. We look forward to providing you with the very best oral surgery care in a comfortable, professional, and warm environment.

Financing & Insurance

At Dental Implant & Oral Surgery Specialists of Iowa, we deliver the finest care at the most reasonable cost to our patients; therefore, payment is due at the time service is rendered unless other arrangements have been made in advance. For your convenience, we accept Visa®, Mastercard®, Discover®, and CareCredit. If you have questions regarding your account, please contact us. Many times, a simple telephone call can clear up any misunderstandings.

Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Our practice makes every effort to provide you with the finest care and the most convenient financial options. To accomplish this, we work with you to maximize your insurance reimbursement for covered procedures. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to any actual procedures.

Please bring your insurance information to your first office visit so that we can expedite reimbursement.
